What is Failure reporting, analysis, and corrective action system?  

A failure reporting, analysis, and corrective action system (FRACAS) is a process for reporting, classifying, analyzing failures and defining and implementing corrective actions in response to those failures.  

It is typically used in an industrial environment to collect data, record and analyze system failures and should be incorporated on the EAMS or CMMS to support reliability improvement. 

FRACAS records the problems related to a product or process and their associated root causes and failure analyses to assist in identifying and implementing corrective actions. 

 

Failure reporting, analysis, and corrective action system training course 

The participants will attend a theoretical training session in FRACAS and a workshop that will allow the participants to implement a FRACAS on an asset problem. 

The FRACAS training workshop will be performed using specific templates and a benchmark example.
